London Borough of Tower Hamlets Council has been sentenced after a five-year-old girl died when playground equipment collapsed on top of her at Mile End Park.
The equipment had not been inspected for over 20 months and the investigation by the HSE found that if regular inspections had been carried out any defects could have been noticed and rectified, preventing the rotten equipment from falling on her.
